I have been baptized in water by immersion in the Name of the Father, Son, and Holy Spirit.

Holiness

I desire to live a holy life, separating myself from worldly practices that defile the body, mind, or spirit, recognizing that my body is the temple of the Holy Spirit.

I agree with and will adhere to the church's Tenets of Faith as set forth in the church Bylaws.

Cooperation

I am willing to cooperate in the work and ministry of this church, be governed by its Bylaws, and contribute regularly to its financial support as the Lord prospers me.

Age Requirement

I affirm that I am at least eighteen (18) years of age.

Attendance Requirement

I affirm that I have regularly attended and supported this church for at least three (3) consecutive months prior to this application.
